महत्वपूर्ण टीप - • ऑनलाइन आवेदन के साथ अभ्यर्थी की जाति जन्मतिथि, स्थानीय निवासी आदि सम्बंधी प्रमाण-पत्र नहीं लिए जा रहे हैं। अभ्यर्थी के सभी प्रमाण पत्र का परीक्षण नियुक्तिकर्ता प्राधिकारी द्वारा नियुक्ति के पूर्व की जाएगी। अभ्यर्थी द्वारा दी गई गलत जानकारी के लिए व्यापन जवाबदार नहीं होगा । इसका संपूर्ण उत्तरदायित्व उसका स्वयं का होगा ।
• OMR उत्तरशीट में परीक्षा दिवस के दिन कोई भी त्रुटि सुधार की सुविधा नहीं दी जाएगी । आवेदन पत्र भरने से पहले विभाग द्वारा जारी विज्ञापन को भली-भांति पढ़ लेवें, आवश्यक दिशा निर्देशों को अत्यंत सावधानी से पढ़ें तथा सेम्पल फार्म (नमूना आवेदन पत्र ) का अवलोकन करें । .
भरे हुए ऑनलाइन आवेदन का प्रिंट आउट अवश्य लेकर रखें। कोविड 19 महामारी के कारण भारत सरकार / राज्य शासन द्वारा समय-समय पर जारी दिशा-निर्देशों का पालन सभी अभ्यर्थियों को करना अनिवार्य होगा |
महत्वपूर्ण जानकारी
1. पात्रता :- आवेदन हेतु पात्रता के संबंध में विभागीय नियम देखें केवल वे अभ्यर्थी पात्र होंगे जो विभागीय नियमों में दी गई शर्तें पूरी करते हों मात्र आवेदन पत्र प्रेषित करना अथवा परीक्षा में सम्मिलित होना अभ्यर्थी को पात्र नहीं बनाता है।
2. ऑनलाइन आवेदन करने की विधि :
ऑनलाइन आवेदन करने की विधि की जानकारी व्यापम की वेबसाइट के लिंक पर उपलब्ध फाइल से प्राप्त की जा सकती है ।
पत्र दिन पूर्व टिकार्ड की पर करने के लिए Registration ID डालकर इसे ऑनलाइन है [भी] [] कार्डको लकरके इसे प्रस्तुत करने की जरूरत हो सकती है। एडमिट कार्ड की द्वितीयम्म द्वारा प्रदाय नहीं किया जायेगा।
4: ऑनलाइन आवेदन भरने में किसी प्रकार की दिक्कत होने पर कार्य दिवसों में 10:00 से कार्यालयीन समय पर संपर्क कर सते कहैं।
